The program’s mission is to provide education which will empower those who have been impacted by diabetes to achieve improved outcomes and heal their lifestyles. Education is provided on an individual and in group settings. Instruction covers the areas of: Diabetes Process Nutrition Physical Activity Medications Pre-conception care, pregnancy and gestational diabetes Monitoring/using results Acute Complications Chronic Complications Goal setting and problem solving Psychosocial adjustmentDiabetes now affects 21 million Americans and more than 6 million of those people do not know they have diabetes. Another 41 million people are estimated to have pre-diabetes, a condition that increases the risk of developing type II diabetes, the most common form of the disease, as well as heart disease and stroke.
